Mission

The Elder Medical Emergency Assistance Program is to aid Tribal Members elders when they are hospitalized.

Eligibility Requirements

  1. Puyallup Tribal Member elder 55 years or older; and
  2. Hospitalized; admitted (1) day or more

Elder Medical Emergency Travel Assistance is granted one time per admittance to hospital.

Application process

The Elder, Elder Designee or Power of Attorney must:

  1. Fill out an Elders Medical Emergency Assistance application. If elder is out of area, an application may be faxed or mailed;
  2. Request that designee or Power of Attorney fill out application for eligible Elder. An application may be faxed or mailed if the applicant is out of state or out of the area.

Program coordinator will:

  1. Contact the hospital to verify that the Elder has been admitted; and
  2. Process request for Elder Medical Emergency Assistance

Elder Medical Assistance:

The amount of assistance is $500.

Other information

The Elder Medical Assistance Program is funded by the Puyallup Tribe. At any time the amount of funding can be either decreased or increased based on funding allocations.
